This workshop provides participants the basic skills necessary to conduct an effective accident investigation and determine the root causes of accidents. The course reviews basic accident investigation procedures and describes accident analysis techniques with emphasis on the use of Root Cause Analysis. Participants will use systems thinking and related tools and techniques to identify contributing and causal factors that underlie variations in performance associated with adverse
events or close calls. The course integrates a number of different analysis methods using actual case studies to demonstrate their advantages, disadvantages, and applications. Participants will engage in a variety of instructional activities including lectures, group discussions, quizzes, and practical exercises to reinforce their understanding and use of Root Cause Analysis.
Notes:
This training will be conducted via Microsoft Teams, and no software download is required. Participants must have a PC or Mac computer with audio and video capabilities, and an uninterrupted internet connection. Participants must present a government-issued photo ID to attend this training.
